- Medstream.global currently operates on a self-pay model. You are responsible for the full consultation fee at the time of service.
- We will provide you with a detailed invoice/receipt, which you may submit to your insurance provider for potential reimbursement.
- Any reimbursement depends on your individual insurance policy. We strongly encourage you to contact your insurance provider directly before your consultation to understand your benefits for online medical services.
Consent to Telehealth Consultation & Data Processing
I, the undersigned patient (or legal guardian/representative if applicable), hereby declare and consent to the following:
- I confirm that the information provided in this form is true, accurate, and complete to the best of my knowledge. I understand that withholding or providing inaccurate information may jeopardize my health and the effectiveness of my online consultation and treatment.
- I consent to receive online medical consultation and/or treatment from the healthcare professionals at Medstream.global via telehealth platforms.
